The short answer
There is no single national UHID card you can download. A UHID (Unique Hospital ID) is issued by each hospital when you first register there, and there's no central "UHID card" button on any official site we checked. What you can download is the ABHA card, which is national. And what most people really want is one of two things: their hospital's lab reports, or a copy of their registration card.
The rest of this guide shows what actually works, with screenshots of the official pages. We checked them on 21 September 2026 and stopped at every login, OTP and captcha screen, so nothing behind those gates is described as tested.
Which card do you actually mean?
Most "UHID card download" searches mix up three different things.

How to get a UHID
A UHID is created by the hospital, not by you online. There are three ways it usually happens.
- At the registration counter. Carry a photo ID and give a mobile number you'll keep. The hospital creates your UHID and gives you an OPD slip or card.
- After an ORS appointment. On the ORS portal you can book an appointment at AIIMS, central government hospitals and many state hospitals. You register with a mobile number or ABHA. The hospital still issues its own registration number.
- With ABHA Scan and Share. In hospitals that support it, you scan the hospital's QR code in an ABHA-enabled app, consent to share your profile, and get a token number. SGT Hospital's published steps are: download the ABHA app, create your ABHA if you don't have one, choose Scan QR Code, scan the hospital's ABDM QR and tap Get Token, agree to consent, then hand the token to the billing executive at the counter. It speeds up registration. The hospital still issues its own UHID or registration number.
One detail that matters later: the mobile number you give at registration is usually where your OTP goes for reports. RML Hospital's official tutorial says to give the correct mobile number when the OPD slip is generated and to keep using the same slip and number on every visit.
Lost your UHID? Where to look
- The OPD slip or registration card. At RML the UHID is printed at the top left of the OPD slip.
- Discharge summaries and bills. Hospitals usually print the ID there too.
- The counter. Ask with a photo ID. Don't register again, because that creates a second record.
How to check reports online
The steps depend on the hospital. Here are the ones we could inspect.
AIIMS Delhi, RML and other central hospitals: the ORS portal
AIIMS's own online OPD page points patients to ors.gov.in, and RML's "Online Registration and Lab reports" menu item redirects there too. The ORS home page has an Other Facilities section with four tiles: e-OPD Card, Payments, Lab Report and Blood Availability.

- Open ors.gov.in/orsportal and click the Lab Report tile.
- On the login page choose Using ABHA (Health ID) or Using Mobile Number.
- On the ABHA tab, enter your ABHA number or address, type the captcha characters and click Proceed.

This has changed. Older blogs, and a 2017 RML tutorial, describe a flow where you pick the hospital, type your UHID and get an OTP. RML's PDF is dated 2017, so its screens are old. Today's Lab Report tile leads to the ABHA or mobile login above. A separate UHID page still exists: ORS "Patient With UHID" asks you to select a state and hospital, enter your UHID and a captcha, and click Proceed. It's labelled for appointments and health records. We didn't verify what it shows after login.
AIIMS lab reports. We didn't find a lab-report page on aiims.edu itself, and the AIIMS "Patient Portal" link returned an error page when we tried it. So we can only say that AIIMS points patients to ORS. We couldn't confirm AIIMS's UHID length from an official page, whatever other blogs claim.
PGIMER Chandigarh: it uses a "CR No."
PGIMER doesn't use the word UHID. Its ID is the CR No. Its official page lists nine steps for OPD patients. In short: the doctor fills the test form, you pay at the fees counter if you're a paying patient, samples are collected, and you wait about 1 to 3 days. Then you go to pgimer.edu.in, click the Lab Reports icon, enter your CR No., click Generate OTP, and enter the OTP sent to your registered mobile.

Limits on the page itself: it shows reports from the last 30 days, for Biochemistry, Biophysics, Endocrinology, Parasitology and Virology labs, for OPD patients. The OTP is only sent if the reports are ready. The downloaded reports are computer-generated and "not valid for medico-legal purpose".
Kerala eHealth: a 16-digit UHID
Kerala's citizen portal at ehealth.kerala.gov.in/portal has two ways in: Using Mobile Number (Send OTP) and Using UHID, which asks for a 16-digit UHID, a password and a captcha.
First time on the UHID login? The portal's own instructions say to click Generate / Reset Password, enter your UHID, Aadhaar number and the mobile number given when the UHID was created, plus the captcha. A default password is sent to your registered mobile. Log in with it, then change it.
Private hospitals and lab chains
Most large private hospitals and diagnostic chains have their own app or patient portal. We haven't checked them, so we're not going to invent steps. Ask the registration desk which app or website they use, and log in with the mobile number you gave at registration.
How to download your ABHA card
If the card you want is the national one, that's the ABHA card. The official ABHA registration page lets you create an ABHA number with Aadhaar or a Driving Licence, or log in if you already have one. Our ABHA login and card download guide walks through it, including the OTP and Aadhaar problems.

UHID login problems and fixes

Stay safe: official sites only
- Type the address yourself: ors.gov.in, abha.abdm.gov.in and hospital websites that end in .gov.in or .edu.in. Lookalike "AIIMS appointment" sites exist.
- Never share your OTP with anyone, including someone claiming to be from the hospital.
- Don't pay a third party to "download your UHID card". We found no official page that charges for it.
Keep your UHIDs and reports together
After a few hospitals you'll hold several IDs: a UHID at one place, a CR No. at another, plus your ABHA. The Ayu app lets you save each hospital's ID next to its reports and prescriptions, so a family member or doctor can find them without a portal hunt.
